How the four differ in practice
Boulevard
Names HIPAA, SOC 2 and PCI Level 1 right on its pricing page. It has a built-in AI receptionist that answers calls and books 24/7, plus a center for payment disputes and chargebacks. QuickBooks is a separate $45/month add-on. Not for: a spa with more than 5 professionals at one location that wants to stay on the entry tier.
Zenoti
Built to run the whole spa: booking, payroll, stock and point of sale. It's sold as a way to replace 8 to 12 separate plans with one. Its AI tools stand out on paper, but they sit in a separate paid 'AI Plus package', and voice and SMS bill by usage on top of the plan. Not for: a spa that wants AI features and a posted price without a custom quote.
Vagaro
The clearest pricing of the four, and the only one where you can buy at a posted price without a sales quote. Its headline rate is a 6-month promo that goes to $30/month after. It's built as a general booking tool for beauty and wellness, not an EMR made for med spas. Not for: a med spa that wants charting as deep as Aesthetic Record's.
Aesthetic Record
A full EMR with high-res photo storage and procedure charting on every tier. That makes it as much clinical-record software as booking software, unlike the other three. The headline price per user is real but not the whole cost: a required $399 setup fee, and AI charting and e-prescribing are add-ons. Not for: a spa that expects the $15-19 figure to be the full monthly cost.

Choose Zenoti if you run many locations and want one platform for booking, payroll and stock. Choose Aesthetic Record if you want full clinical charting on every tier.
